 Dear Grant Requestor, This letter and the following Educational Grant Request (EGR) form are intended to provide information for a grant request in support of continuing medical education (CME) to be considered by P&G Pharmaceuticals. Ordinarily, especially for local or regional CME, grant requests should be made through your local P&GP Medical Science Liaison. Otherwise, EGR forms should be submitted by fax at least six weeks prior to the CME activity to the P&GP Grant Office. (513) 277-8709. P&G Pharmaceuticals is pleased to collaborate with accredited CME providers to provide funding and to satisfy other unsolicited requests for assistance from the provider as allowed by the FDA Guidance for Industry-Supported Scientific and Educational Activities, the ACCME Standards for Commercial SupportSM, the PhRMA Code on Interactions with Healthcare Professionals and the OIG Compliance Program Guidance for Pharmaceutical Manufacturers. Specifically, P&G Pharmaceuticals will consider EGRs from accredited CME providers to help meet medical educational needs in the following diseases and conditions only: Osteoporosis Inflammatory bowel disease Overactive bladder Simply click on the link at the bottom of this page, print out the EGR form, complete it, and return by fax. Should you need additional space than the EGR form allows, e.g., for budget itemization or description of enduring materials, please fax a second page with the additional information. If you have any questions or need clarification, please e-mail wilson.fs@pg.com. The CME contact listed on the EGR will be informed of the acceptance of their request within two weeks. If accepted, the provider will need to submit (one time only) a copy of their current W-9 form as well as complete the sections marked "Optional," including the "Wire Transfer" section of the EGR if wire transfer is the desired form of grant payment.
